Can I automatically fill in the initiator’s name, department and position?
Yes, you can. First, you’ll need to create a lookup table with these fields (employee number, name, department and position). Ensure that your lookup form name is Employees. In your workflow, go ahead and create fields for displaying the employee number, name, department and position. Note that these field names do NOT have to match but ensure that you have one field Employee No for storing the employee number.
Create a lookup mapping based on employee number as the key field as shown in the screenshot below.

Now, create a new request and you’ll see that these 4 fields are automatically populated.
